IMPORTANT INFORMATION FOR TAXPAYERS THAT HAVE BEEN CONTACTED BY CCA.
Based on an income tax information sharing agreement with the Internal Revenue Service and CCA membership agreements
with the communities listed below, CCA has obtained federal income tax information to identify taxpayers from these communities:

• Englewood
• Hamilton
• Lakewood
• Lancaster
• Lima
• Lorain
• Salem
• Stow
• Trotwood
• Troy
• Warren
• Wickliffe
These communities have become members of CCA so that CCA may obtain and utilize federal tax information on their behalf.
Please be advised that under federal law the communities are not authorized
to obtain federal tax information from the IRS on their own.
CCA is notifying the taxpayers of these communities that have filed federal income tax returns but have not filed
city tax returns or may have underreported city taxable income.
You must respond directly to CCA if you have received a notice from CCA.
The communities listed above are not authorized to answer questions or process
income tax return information based on tax information obtained from the IRS.

with any questions regarding this program or the information requested in the notice

Information about CCA’s Special Member Program
CCA provides compliance and delinquency programs to its Special Members
primarily by utilizing IRS information to identify taxpayers that have not filed
required municipal income tax returns or that have unreported municipal taxable income.
Special Member programs are limited to agreed-upon compliance and delinquency procedures
in each CCA Special Member contract.

Assistant City Prosecutor Final Notice online payment help
To make a Final Notice payment online: login to eFile,
select form type Penalty & Interest Assessment then [ Next ].
Select your City Code, [ TAB ], set Period End to 12/11, Description to 20, Form Type to 98,
enter amounts you are paying for Tax Due, Penalty and Interest,
check box to sign and [ File ] to continue the payment process.
